Summary

The Licensed Practical Nurse (LPN) contributes to whole person healthcare by providing evidenced-based nursing skills to care for patients in both the Withdrawal Management and Intensive units.

Essential

Duties & Responsibilities

Timely administer medications to patients and monitor patients for reactions or side effects Maintain accurate, detailed reports and records Record patients' medical information and vital signs. Monitor, record, and report symptoms or changes in patients' conditions. Receive and transcribe written orders from the medical and psychiatric provider Medication Reconciliation process Facilitate effective crisis intervention Assist the RN in consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referrals to other levels of care when appropriate Assist the RN in modification of the patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes Direct or supervise residential technicians while on shift Provide medical re-education to patients as needed Performs related duties as required. *All agency staff are required to attend all mandatory department/agency meetings and trainings*
