About the role

Provide direct care to patients including ADLs, medication administration, and mental/physical status assessments. Responsible for room preparation, discharge processes, and communicating with patients and their families.

Capitol View Transitional Care Center is a state-of-the-art true transitional care unit (TCU) located in Regions Hospital (we are part of the HealthPartners organization as a separate nonprofit organization.) We provide quality care to patients from 18+ years of age who are in need of a short-term stay.

Our average length of stay is 17 days.

As a RN/LPN your patient to nursing staff ratio is three times lower than the average nursing home.

In this role you will be responsible for the following job

Duties

  • Provide direct care to patients by providing ADLs, accessing physical and mental status of patient to determine efficacy of treatment and care plan, administer medication complete all documentation, etc.
  • Prepare room preparation and room discharge of patients
  • Answer call lights and provide follow-up to ensure needs are met.
  • Answer questions from patients and families or direct them to the appropriate person.
  • Performs other duties as assigned. FTE: 0.0; Per Diem/Casual Work Schedule: Required to work a minimum of 4 shifts per month - 2 shifts must be a weekend shift.

Salary

  • LPN starting rate at $28.29/hour. Pay rate is based off previous LPN work experience. RN starting rate at $36.47/hour. Pay rate is based off previous RN work experience.

  • Graduate of an accredited school of nursing
  • Currently licensed as a LPN or RN with the Minnesota Board of Nursing
  • Current CPR Card Certification
  • Must be able to attend and have reliable transportation to and from HealthPartners' Nursing Orientation in Bloomington on Tuesday and Wednesday from 8am-430pm.

Requirements

Must be a graduate of an accredited nursing school and currently licensed as an RN or LPN with the Minnesota Board of Nursing. A current CPR certification and reliable transportation for orientation are required.
