About the role

The Coordinator provides recovery support and harm reduction services to improve patient access to substance use treatment and primary care. They act as a liaison between patients and healthcare providers while conducting education and training on overdose prevention and MAT.

Sun River Health is a federally qualified, nonprofit health center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia, and Westchester counties. Sun River Health seeks a full-time Overdose Prevention Coordinator to join our Hudson Valley Poughkeepsie site.

Position Summary

The Overdose Prevention Coordinator provides recovery support services using a harm reduction approach to engage patients and improve access to substance use treatment, primary care, and community-based resources. The Coordinator serves as a liaison between patients, healthcare providers, and community organizations, helping individuals navigate care while promoting overdose prevention, health education, and recovery support. The position supports Medication Assisted Treatment (MAT) initiatives and connects patients to internal and external resources that improve health outcomes.

Pay Range

$20.00 - $23.13 per hour

Sign-On Bonus: $1,500 Sign-On Bonus for Eligible External Applicants – Terms and Conditions Apply

Essential Duties and Responsibilities

Provides recovery support services using a harm reduction approach.

Engages patients and links them to substance use treatment, primary care, behavioral health services, and community resources.

Educates patients regarding Medication Assisted Treatment (MAT), Hepatitis C (HCV), overdose prevention, risk reduction strategies, and medication adherence.

Conducts trainings and presentations on MAT, Naloxone, overdose prevention, and harm reduction resources.

Serves as a regional community and site liaison to expand access to substance use and primary care treatment services.

Coordinates referrals and facilitates connections to internal and external support services.

Assists patients with navigating healthcare and social service systems.

Collaborates with multidisciplinary teams to support patient-centered care.

Maintains accurate documentation and ensures compliance with program requirements and confidentiality standards.

Participates in community outreach activities and promotes awareness of available recovery support services.

Performs all related duties as assigned and supports organizational needs during emergencies.
