  • Summary: Registered Nurse (RN) Care Manager manages high-risk patients who are dealing with addiction and collaborates with the care team to develop and update an individual care plan, manage medications, and support patients. The RN Care Manager serves as an advocate for an individual’s healthcare needs and is dedicated to enhancing the health status of the community. Although focused on high-risk patients, the working RN is a part of the overall medical team.
  • Duties/Responsibilities: Partners with Providers, LPNs, and Medical Assistants in client/patient care. Assists with the identification of patients with special health care needs/chronic diseases. Develops a method of tracking patient compliance with clinical guidelines and the plan of care. Uses an empathetic and positive approach to engage clients in shared decision-making and in collaboration to develop care plans. Ensures patient confidentiality as defined by HIPAA and the medical facility. In partnership with didgwalic Leadership, develop objective measures of performance with a clear process for implementing CQI (Continuous Quality Improvement). Develops, conducts, and evaluates formal or informal in-services and programs with a focus on the highest risk groups Offer one-on-one educational support for each risk group or an individual patient, tied to best practice methodology, Addiction Care Models, and trauma-informed care. Partner with all departments to ensure that clients’ needs are met (e.g., housing, food insecurity, mental health) Develops and maintains client files to meet all local, state, and federal program requirements. Integrate harm reduction principles and strategies to mitigate health risks and improve patient outcomes. Other duties as assigned by the Nursing Manager.
  • QualificationsRequired
  • Skills/Abilities: Knowledge of the current medical practices in primary care, mental health, and addiction medicine. Knowledge of the information and techniques needed to diagnose and treat injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures. Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction. Good empathy, as well as personal and ethical boundaries. Excellent attention to detail and ability to balance competing priorities. Ability to work in a cross-cultural environment, understands the social and cultural context of the patients at the didgʷálič Wellness Center. Understanding the role of trauma, historical, community, family, and personal in wellness and recovery. Understands and adheres to state (RCW, WAC) and federal confidentiality regulations (42 CFR part 2). Effective communication; written, verbal, and interpersonal skills, including conflict resolution. Basic computer and software experience using MS Office products.

  • Experience: Must be a Licensed Registered Nurse in good standing with the State of Washington. Five years’ experience of professional work as a Registered Nurse is desirable. Bachelor of Nursing (BSN) preferred. Previous substance abuse treatment and/or primary care experience strongly preferred. Addiction Nursing Certification, or obtained within 1.5 years of employment in this role.

  • Requirements: Prolonged periods sitting at a desk and working on a computer. Prolonged periods of standing and bending. You must be able to lift 50 pounds at times. Frequent exposure to chemicals, bloodborne pathogens, and patient body fluids.

Requirements

Candidates must be a Licensed Registered Nurse in Washington with BLS certification and preferably a BSN and five years of experience. Experience in substance abuse treatment or primary care is strongly preferred, and addiction nursing certification is required within 1.5 years.
