Providence Hospice is seeking a RN Hospice Case Manager, Full Time in St.
John, IN and throughout Lake County.
We are looking for a compassionate RN to join our established, experienced hospice team.
We provide exceptional patient centered care to patients and families facing end of life.
Monday - Friday. This position requires (1) on call night shift weekly.

Active Registered Nurse (RN) License in the state of Indiana. At least 1 year of RN experience, preferably in Hospice or Home Health. Drivers License in good standing. Reliable transportation.
