The Hospice RN provides quality field care and case management for patients in residences and facilities to maximize comfort and quality of life. They supervise field staff and collaborate with an interdisciplinary group and physicians to implement nursing care plans.
Reporting to the Clinical Care Manager, the Hospice RN- Field is responsible for providing quality care to patients with Gulfside Healthcare Services, in the field.
The field includes patients who reside in private residences, assisted living facilities, and skilled nursing homes.
The Hospice RN utilizes assessment, intervention, and education to maximize patient comfort and to enhance the quality of life for the patient/family.
The Hospice RN- Field oversees and case manages the needs of the patients assigned to them.
This includes providing supervision for the L.P.N.’s, C.N.A.’s, and other assigned staff in the field.
The Hospice RN- Field is responsible for maintaining his/her own clinical and mandatory education requirements and for meeting the established standards of their profession as well as those accountable for his/her practice, assigned duties/patient care, and is an advocate for patients/families and their profession.
