Responsibilities

  • Evaluate and treat residents requiring physical therapy within geriatric rehab.
  • Develop and implement individualized plans of care based on evaluation findings.
  • Administer therapeutic exercises, gait training, manual therapy, modalities, and functional activities.
  • Reassess progress regularly and adjust the plan of care to achieve goals.
  • Document assessments, treatments, and progress accurately in the resident’s records.
  • Collaborate with an interdisciplinary team including therapy, nursing, and medical staff to coordinate care and discharge planning.
  • Ensure compliance with regulatory standards, patient privacy (HIPAA), and facility policies.

Requirements

  • Active Physical Therapist license in Maryland or eligibility for licensure in Maryland
  • Doctor of Physical Therapy (DPT) degree or equivalent
  • Strong clinical skills in geriatric rehab and skilled nursing facility settings
  • Excellent communication and documentation abilities
  • Ability to work 35 hours per week in a 5x8 day schedule
  • Willingness to travel for a temporary assignment
