We are seeking a compassionate and collaborative Nurse Practitioner or Physician Assistant to join CS360, an innovative ambulatory intensive care program designed to care for our highest-risk patients.

About CS360 CS360 functions as an "ambulatory ICU," bringing together a multidisciplinary team to proactively manage complex medical conditions, optimize medications, coordinate care, and improve patient outcomes.
This unique, consultative program focuses on individuals with complex chronic conditions, frequent hospitalizations, and high healthcare utilization, helping them remain healthy and safely at home while reducing avoidable hospital admissions.
Our collaborative team includes: Physicians and Advanced Practice Providers Clinical Pharmacists Care Managers and Social Workers Flow Managers and Administrative Support Together, we take a comprehensive approach to caring for some of Cedars-Sinai's most medically vulnerable patients.
Requires current licensure as a Nurse Practitioner or Physician Assistant and DEA certification. Preferred candidates have 2+ years of clinical experience and proficiency with Epic electronic medical records.
