DEPARTMENT DESCRIPTION Harborview’s Pediatrics Clinic provides primary care for infants, children and teens. The clinic utilizes a multidisciplinary staff that is highly collaborative and supportive, working together to achieve the best patient outcomes possible.
Collaborate with the interdisciplinary team to identify engage, assess, plan, implement, engage, assess, plan, implement, coordinate, monitor, and evaluate options and services in meeting the needs of newly arrived pediatric patients Work closely with the clinic, community agencies, schools, hospitals, and function across the continuum of care, interfacing with patients, caregivers, providers, and community resources to facilitate the best possible clinical outcomes and optimal level of wellness while using resources appropriately Coordinate and manage the care of newly arrived medically complex children for up to one year (including close partnership with resettlement agencies and Seattle Children's) Role includes coordination of the following: DME, referrals to SCH and other community medical organizations.
