The schedule is 8:00 a.m. to 4:30 p.m., typically five eight-hour days with a 30-minute lunch.

This role supports rehabilitation services for residents in a long-term care environment.

Responsibilities

  • Conduct initial evaluations and ongoing assessments of residents to develop and update individualized plans of care.
  • Deliver physical therapy interventions, including therapeutic exercises, gait and balance training, mobility assistance, and range-of-motion activities.
  • Reassess progress regularly, document outcomes, and modify care plans to meet resident goals and safety standards.
  • Collaborate with an interdisciplinary team (nursing, OT, SLP, and therapy assistants) to coordinate resident care and discharge planning.
  • Maintain accurate, timely documentation in accordance with regulatory and facility requirements and ensure patient safety during all therapies.
  • Educate residents and families on home exercise programs and post-discharge strategies; contribute to transition planning as appropriate.

Requirements

  • Active Physical Therapist license in New Mexico (or eligible for NM licensure)
  • Doctor of Physical Therapy (DPT) degree from an accredited program
  • Ability to practice in a skilled nursing setting and to collaborate within a multidisciplinary team
  • Prior experience in geriatric or long-term care rehabilitation preferred
  • Willingness to participate in a travel assignment and adapt to a new care setting
