Provide direct behavioral health services including therapy and post-hospitalization assessments for recently discharged members. Manage a rolling caseload to conduct outreach and identify members at risk for relapse or readmission.

More than 700 physicians and 300 associate practitioners work together in a unique integrated care model to provide high-quality care to Kaiser Permanente members.

Our focus is on long-term health, effective prevention, diagnosis and treatment of disease.

Care is delivered across 40+ specialties at 26 medical offices featuring state-of-the-art equipment, labs, imaging services, and pharmacies.

We also provide surgical services and around-the-clock care at some of the area’s top hospitals.

We invite applications for a licensed Behavioral Health Transitional Case Manager LCSW to join our busy practice located in metropolitan Atlanta.

What You Can Expect Provides direct behavioral health services within scope of practice, including post-hospitalization assessments, individual therapy, group therapy, and other clinically appropriate interventions Maintains a rolling caseload of recently discharged members and conducts timely outreach, including multiple contact attempts as appropriate, to engage members and caregivers following discharge Completes comprehensive assessments of members' psychosocial, behavioral health, environmental, financial, and functional needs; evaluates readiness for self-management and identifies factors that may affect treatment engagement outcomes Utilizes clinical judgement to identify members at risk for relapse, deterioration, avoidable emergency department utilization, readmission, or disengagement from treatment, and escalates concerns to the appropriate care team members Minimum Requirements Licensed Clinical Social Worker 2 - 5 years inpatient psychiatric care 2 - 5 years discharge planning experience and/or transitional case management or complex case management experience working with behavioral health patients TSPMG Clinicians Are committed to providing care of the highest quality Practice evidence-based medicine Provide excellence in service Are honest and ethical and show integrity in their actions Demonstrate accountability for their performance Dedicated to the success of the team Entrusted with maintaining the affordability of care We Provide You Comprehensive electronic health record (Epic) and AI assistance to streamline coordinated care Competitive compensation based on skills, qualifications, certifications, training, and market data Opportunities for professional development Comprehensive benefits including medical and dental insurance, pension plan and 401(k), life insurance, and generous paid time off Complete professional liability coverage, including tail coverage, provided at no cost Reimbursement for board certification and continuing medical education In-house CME opportunities Additional benefits supporting work/life balance Equal Opportunity Employer TSPMG is an equal opportunity employer.

Requirements

Requires a Licensed Clinical Social Worker (LCSW) license. Candidates must have 2-5 years of experience in inpatient psychiatric care and discharge planning or complex case management.
