Conduct comprehensive nursing assessments during the Home Care/Hospice Admission Visit Review and correctly implement the patient’s plan of care, as established by the Physician Identify and initiate appropriate interventions for immediate care needs during the admission process Confirm eligibility for Home Care and Hospice services based upon regulatory requirements Observe proper infection control and safety procedures Supervise health assistant services Coordinate care team to meet patient needs Position Type This is a casual position.QualificationsQualifications Current Registered Nurse (RN) license in the State of Minnesota. Valid Minnesota driver’s license and current auto insurance for travel between patient homes. Ideal Candidate Experience in home health or hospice nursing preferred. Strong patient assessment, care planning, and clinical decision-making skills. Ability to educate patients and families on disease processes, symptom management, and end-of-life care. Comfortable working independently in home environments while collaborating with an interdisciplinary care team. Strong organizational, communication, and documentation skills, including proficiency with electronic medical records. Knowledge of home care and hospice regulations and best practices.
