The RN Care Manager plans and delivers patient care using the nursing process of assessment, planning, intervention, and evaluation. They interact with patients, families, and interdisciplinary teams to maintain professional nursing standards.

What we are looking for--An RN who plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, families and other interdisciplinary team members while maintaining standards of professional nursing. This position services Lancaster and surrounding areas.
30pm with occasional holiday and weekend rotation.

Because you're committed to providing superior care, you understand the importance of being properly prepared for you role at Ohio's Hospice. That's why you'll bring to your role the following set of qualifications:
Graduate of an accredited nursing school; ASN; BSN preferred.
Active RN license in Ohio without any board actions.
Two years acute care nursing experience preferred.
Certification Hospice & Palliative Medicine (CHPN) preferred
CPR Certified.
Computer skills sufficient to properly document services and care.
Able to travel during daytime, nighttime or inclement weather.
Valid Driver's License with Safe Driving Record.
State minimum Automobile insurance coverage.

Candidates must be a graduate of an accredited nursing school with an active Ohio RN license and CPR certification. A BSN, two years of acute care experience, and CHPN certification are preferred.
