About the role

The Case Manager helps individuals with substance use and mental health disorders by connecting them with essential community resources and basic needs. They coordinate referrals, collaborate with multidisciplinary teams, and maintain accurate client documentation in electronic health records.

Overview

On Demand Counseling is seeking a compassionate, organized, and motivated Case Manager to join our Newton Falls team. This position plays a vital role in helping individuals with substance use and mental health disorders overcome barriers to treatment and recovery by connecting them with the resources and support they need to succeed.

Responsibilities

Assist clients with obtaining and maintaining basic needs, including food assistance, Medicaid, housing, identification documents, transportation, and other community resources. Coordinate referrals to medical, behavioral health, vocational, and social service providers. Collaborate with counselors and the multidisciplinary treatment team to support client progress. Assist with discharge planning and linkage to ongoing community supports. Advocate for clients and help them navigate complex systems and services. Maintain timely, accurate documentation in the electronic health record. Monitor client progress toward case management goals and follow up on identified needs. Develop positive working relationships with community agencies and referral partners.

Qualifications

Must be at least 18 years of age. Successful completion of BCI/FBI background check and pre-employment drug screen. Minimum of a high school diploma or equivalent CDCA preferred (or willingness to obtain). We are happy to help the right candidate become credentialed.

Skills/Experience

Experience working with individuals affected by substance use disorders, mental health disorders, or other vulnerable populations is preferred. Strong communication, organization, and problem-solving skills. Ability to work independently while collaborating effectively within a multidisciplinary team. Valid driver's license and reliable transportation preferred.
