What You'll Do As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey.

Your responsibilities include: Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers Providing ongoing emotional support and counseling as a consistent presence throughout the care journey Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs Guiding families through Advanced Directives with clarity and sensitivity Assisting with facility placement for patients when appropriate Maintaining accurate, timely clinical documentation at the patient's residence and in agency records Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans Participating in on-call duties on evenings and weekends as needed Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures Benefits We Offer Competitive pay Medical / Dental / Vision Insurance plans Long & Short-Term Disability 401(k) with company match Auto Allowance & Gas Card Company provided equipment Paid Time Off Why Choose Transitions Hospice?

Requirements

What You Bring Master's degree in Social Work. Master’s degree from school accredited by the Council on Social Work Education required in states as specified. Licensure and/or certification as required by the State. Minimum of 2+ years social service experience in a health care setting or social services agency. Experience in hospice, palliative care, or a related healthcare setting preferred Deep comfort with grief, loss, and end-of-life conversations Strong knowledge of community resources and care coordination Excellent documentation skills and attention to compliance Self-directed with the ability to work with little supervision Valid driver's license and willingness to travel locally for patient visits
