Per diem Admissions/Team Nurse

The Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.

• Location: Alameda & Contra Costa Counties with occasional coverage to San Mateo County as needed Hope Hospice follows all CDPH vaccine requirements for healthcare personnel.
• Essential
• Duties &
• Responsibilities include (but are not limited to): Make in-person nursing visits to patients as assigned, or as indicated when case managing. Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals. Provide effective management of symptoms, including prompt escalation to providers or others as indicated. Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services. Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status.
• Education/Experience: ASN or Bachelor’s degree in nursing from an accredited School of Nursing. Experience in acute care/ home health and/or hospice setting preferred.

• Registrations: Current license as a registered professional nurse in the State of California. Public Health Nursing certification highly desirable. HPNA certification preferred.
• Pay
