The Admissions/Team Nurse provides coverage for Nurse Case Managers and processes admissions for eligible hospice patients. Responsibilities include conducting in-person nursing visits, managing symptoms, and coordinating care with a transdisciplinary team.

Requires an ASN or Bachelor's degree in nursing and a current California registered professional nurse license. Experience in acute care, home health, or hospice settings is preferred, along with HPNA or Public Health Nursing certifications.
