ABOUT METROCREST SERVICES Metrocrest Services exists to provide hope and build pathways to economic mobility for neighbors in the north Dallas area. We operate a whole-household, SDOH (Social Determinants of Health)-aligned model organized around five pillars: (1) Income (2) Health and Food (3) Workforce (4) Education/Training (5) Housing. Our neighbors are at the center of everything we do.

THE OPPORTUNITY The Health Navigator helps neighbors overcome barriers to health and wellness by connecting them with the care, resources, and support they need to thrive. This role serves as the bridge between healthcare providers, community partners, and Metrocrest's coaching team, ensuring neighbors experience coordinated, compassionate, and neighbor-centered care.

This role primarily works 8

00 AM–5:00 PM, Monday through Friday, with occasional evenings and weekends based on program needs.

Requirements

WHAT YOU'LL DO Connect neighbors to primary care, behavioral health, dental, specialty care, and other healthcare resources Build trusted relationships with neighbors while helping them navigate complex healthcare systems Coordinate referrals and ensure warm handoffs and follow-through with healthcare providers and community partners Develop and maintain relationships with clinics, FQHCs, hospitals, and other healthcare organizations Deliver health education and nutrition programming that supports long-term wellness Coordinate Metrocrest's senior meal program and wellness support for homebound older adults Conduct home visits as needed to assess changing health needs and connect neighbors with appropriate services Track referrals, outcomes, and health-related services in Salesforce Collaborate across Metrocrest's coaching model to ensure healthcare needs are integrated into each neighbor's whole-household success plan WHO YOU ARE Compassionate, culturally responsive, and committed to health equity A trusted relationship-builder who collaborates effectively with neighbors, healthcare providers, and community partners Curious and resourceful, always looking for creative solutions to barriers facing neighbors Detail-oriented and committed to following every referral through to completion Comfortable working independently while thriving as part of a collaborative team Passionate about improving health outcomes through education, advocacy, and connection Committed to Metrocrest's Neighbor-First philosophy and helping every neighbor achieve greater stability and independence WHAT YOU BRING (MINIMUM QUALIFICATIONS) Bachelor's degree in Public Health, Health Education, Health Promotion, Nursing, Medical Social Work, Substance Abuse/Behavioral Disorders, or related field 3–5 years of experience in the healthcare, community health, or social service field Bilingual: English/Spanish required Proficiency with Salesforce or comparable client tracking system; strong data entry skills and attention to detail

Preferred Qualifications

Community Health Worker (CHW) Certification Experience in Medical Social Work Familiarity with SDOH screening frameworks or comparable whole-household assessment tools Experience working with uninsured or underinsured populations and navigating low-cost or free care options

SALARY Salary for this FLSA-exempt position is commensurate with experience
