Document patient information, including assessments, interventions, and outcomes, accurately and timely in electronic medical records. Communicate effectively with residents, families, and healthcare team members to provide updates on patient status and address any concerns or questions. Participate in interdisciplinary care conferences and contribute to care planning and coordination. Adhere to all facility policies and procedures, as well as state and federal regulations governing nursing practice. Maintain a clean and organized work environment and ensure compliance with infection control protocols. Demonstrate professionalism, empathy, and respect in all interactions with residents, families, and colleagues.
Current licensure as a Licensed Vocational Nurse (LVN) in the state of California. Minimum of 1 year of experience in a healthcare setting, preferably in long-term care or skilled nursing. Strong clinical skills and knowledge of nursing principles and practices. Excellent communication and interpersonal skills, with the ability to build rapport with residents and families. Compassionate and patient-centered approach to care, with a commitment to promoting dignity and independence. Ability to work effectively as part of a multidisciplinary team and collaborate with other healthcare professionals. Flexibility to work various shifts, including weekends and holidays, as needed.
