The Care Navigator acts as a patient advocate and coordinator to manage complex patients and facilitate quality, cost-efficient outcomes. This role involves coordinating and procuring services both within and outside of the hospital.
Care Management - Care Coordinator BASIC PURPOSE OF THE JOB Under the supervision of the Director for Care Management and Coordination or his/her designee, the Care Navigator acts as a patient advocate and care coordinator to hospital clients.
An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes.
Care Navigator will be responsible for coordinating patient’s care within and outside of GRMC.
JOB REQUIREMENTS Minimum Education Healthcare professional with degree in health science or equivalent.
Degree in nursing or social work strongly preferred.
Minimum Work Experience Three (3) to five (5) years experience in care coordination role or clinical practices.
On-island payer knowledge highly desirable.
Experience with manage care or care coordination preferred.
Required Licenses / Certifications Case Management certification preferred.
Requires a degree in health science, with nursing or social work degrees strongly preferred. Candidates should have three to five years of experience in care coordination or clinical practices.
