About the role

Provide direct nursing care to adult patients on a medical-surgical rehab unit, including assessment and medication administration. Collaborate with multidisciplinary teams to implement care plans and document progress using Cerner EMR.

Responsibilities

  • Provide direct nursing care for Med Surg Rehab patients, coordinating medical and rehabilitation needs (physical, occupational, speech therapy) as part of a holistic approach.
  • Manage patient care with a nurse-to-patient ratio of approximately 1:4/5; collaborate with interdisciplinary team to optimize functional recovery.
  • May serve as backup Charge Nurse; coordinate total nursing care, supervise team members, and ensure adherence to professional standards of practice.
  • Administer medications and treatments, monitor vital signs and device therapy, perform routine assessments, and document in the electronic medical record (Cerner).
  • Engage in patient and family teaching, discharge planning, and coordination with therapists to support functional goals and safe transitions of care.
  • May float between Acute Rehab and Med Surg units and to other units as needed within scope of practice.

Requirements

  • Active Vermont RN license or Compact license in good standing
  • BLS (American Heart Association) and ACLS (American Heart Association) certifications
  • Minimum 2 years of recent Med Surg Rehab experience; Charge RN experience required
  • BSN is strongly preferred
  • Prior travel experience (at least 3 months) required
  • Willingness to float to other units as needed within scope of practice
  • Must have own reliable transportation due to community setting
