Responsibilities
- Plan and conduct segments of physical therapy treatment programs to restore and improve physical, social and mental functions, aligning with department objectives.
- Conduct patient evaluations and establish individualized treatment plans using appropriate clinical techniques and tools.
- Provide therapy interventions following established protocols and adjust as needed; establish discharge plans and communicate them to the appropriate parties.
- Collaborate as a co-leader in an interdisciplinary approach to patient groups; participate in coordination with care teams.
- Complete comprehensive and accurate clinical documentation within established deadlines; supervise assigned PT assistants and support staff as appropriate.
Requirements
- Doctor of Physical Therapy (DPT) degree; required (only if the license was obtained on or after January 2016; or foreign equivalent).
- Physical Therapy licensure in all applicable states of practice; required.
- CPR certification or Basic Life Support (BLS) certification from an approved vendor per policy; required.
- Valid Driver’s License and must be at least 21 years of age; required.
- At least two years of recent field experience; current BLS certification; and a current nursing license per the state of assignment.
