As a Medical Case Management Care Coordinator, you'll play an important role in ensuring children receive timely healthcare services and critical case management support. Working closely with caregivers, caseworkers, and healthcare providers, you'll help coordinate essential medical services while making a meaningful impact on the lives of youth and families throughout the Aurora community. Position overview

The Medical Case Management Care Coordinator supports the timely delivery of ongoing medical case management services for youth. This position monitors required healthcare activities, reviews case documentation, requests medical records, prepares case management packets, and maintains accurate information within the HealthWorks Evolve Portal.

The ideal candidate is organized, detail-oriented, and comfortable working with reports, databases, and multiple deadlines while collaborating with healthcare providers, caregivers, caseworkers, and internal team members.

Key responsibilities

  • Medical case management coordination
  • Ensure required medical case management activities are completed within established contractual time frames.
  • Assist with the final review of completed critical case activities.
  • Monitor upcoming medical examinations, Annual Case Reviews (ACRs), and other required services based on funder benchmarks.
  • Complete individualized treatment plans for assigned cases.
  • Follow up on pending assignments, documentation, and barriers affecting timely service delivery.
  • Medical records & provider coordination
  • Contact assigned primary care and dental providers to request medical records.
  • Review received health information for completeness and accuracy.
  • Prepare and distribute Medical Case Management packets containing health information, placement details, and contact information to assigned caseworkers within required timeframes.
  • Coordinate communication with healthcare providers to ensure continuity of care.
  • Communication & case support
  • Generate correspondence to notify caregivers and caseworkers of assigned Medical Case Management providers.
  • Collaborate with caregivers, healthcare providers, and caseworkers to support coordinated care.
  • Communicate concerns, barriers, or delays affecting assigned cases.
  • Documentation & reporting
  • Complete accurate and timely data entry within the HealthWorks Evolve Portal.
  • Maintain complete and accurate case notes for all assigned cases.
  • Review and complete tasks associated with SACWIS and other DCFS reports.
  • Monitor daily, weekly, and monthly reports to ensure compliance with contractual benchmarks.
  • Generate daily production reports detailing completed assignments, pending cases, and identified issues for the Director.

Qualifications

  • Required
  • Bachelor's or Associate degree in Social Work, a computer-related field, or a related discipline, or an equivalent combination of education and experience.
  • Valid Illinois driver's license and current automobile insurance.
  • Strong written and verbal communication skills.
  • Proficiency with databases, Microsoft Excel, Microsoft Access, and other computer applications.
  • Strong organizational skills with the ability to manage multiple priorities and deadlines.
  • Ability to analyze information, solve problems, and maintain accurate documentation.
  • Preferred
  • Bachelor's degree with at least two years of professional experience in social services or a related field.
  • OR
  • Associate degree with at least three years of professional experience in social services or a related field.
  • Experience working with children, families, foster care, child welfare, healthcare, or community-based programs.
  • Experience coordinating healthcare services or reviewing medical records.
  • Familiarity with DCFS reporting systems, SACWIS, or case management databases.
  • Experience tracking performance metrics and contractual benchmarks.

Requirements

Candidates need a Bachelor's or Associate degree in Social Work or a related field, along with a valid Illinois driver's license and insurance. Proficiency in Microsoft Office and experience in social services or healthcare coordination is preferred.
