The Substance Use and STD Navigator provides direct services to identified patients with substance use disorder or sexually transmitted diseases at Community Medical Center's (CMC) Emergency and inpatient units. Develops a plan of care to coordinate outreach screening, assessment, referral, treatment, education, data collection, and utilization of community based resources to facilitate with compliance of the medical care plan. The position requires collaborative efforts with physicians, Case Management, Social Workers, and other internal and external personnel to promote appropriate utilization of healthcare resources.
Requires an Associate's degree and 2 years of experience in substance abuse or STD population assessment. A current BLS certification from the American Heart Association is mandatory.
