About the role

The RN Navigator serves as the primary contact for the nephrology service line, guiding patients through the continuum of care using evidence-based protocols. They coordinate care, perform triage, and act as a liaison between patients, providers, and the multidisciplinary team.

The RN Navigator serves as the initial point of contact for the nephrology service line. This role is responsible for guiding patients through the continuum of care using evidence-based protocols developed in collaboration with the Medical Director. The RN Navigator coordinates care, supports clinical visits, performs triage, provides comprehensive patient and referral source education, and acts as the primary liaison between patients, providers, and the multidisciplinary care team to ensure timely, high-quality, and patient-centered care.

Job Qualifications

  • Required
  • Active SC Registered Nurse (RN) license
  • Minimum of 1 year of clinical nursing experience (women’s health, nephrology, OB/GYN, or related specialty preferred)
  • Strong clinical assessment, triage, and patient education skills
  • Excellent communication and organizational abilities Preferred
  • Experience in care navigation, case management, or care coordination
  • Experience educating patients, primary care providers, or referral partners
  • Familiarity with urogynecologic conditions and treatments
  • Experience with protocol development or quality improvement initiatives
  • Proficiency with electronic medical records (EMR) systems

Requirements

Candidates must hold an active SC Registered Nurse license and have at least one year of clinical nursing experience. Preferred qualifications include experience in care navigation, urogynecologic conditions, and proficiency with EMR systems.
