About the role

The nurse assesses patient care needs and provides skilled nursing and teaching to patients and families in home or community settings. They coordinate medical, social, and financial resources to develop and implement comprehensive plans of care for terminal illness.

Park Nicollet is looking to hire a Registered Nurse to join our Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary

The Outpatient Hospice Case Manager registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.

The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.

The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.

This position will support the West Team (ex: Lake Minnetonka area)

Work Schedule

4 days a week 8a-4: 30p and every 4th weekend

Required Qualifications

  • Education,
  • Experience or
  • Equivalent
  • Combination:
  • Baccalaureate degree in Nursing required.
  • Work experience as a registered nurse for at least two years
  • Licensure/
  • Registration/
  • Certification:
  • Current license to practice as a professional nurse in Minnesota.
  • Knowledge,
  • Skills, and
  • Abilities:
  • Must be able to function independently with minimal supervision.
  • High level of sensitivity, assessment skills, technical-skills, and communications skills.
  • Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.

Preferred Qualifications

Education, Experience or Equivalent Combination:

  • Experience in hospice desirable

Requirements

Requires a Baccalaureate degree in Nursing and at least two years of experience as a registered nurse. A current professional nursing license in Minnesota is mandatory.
