Position Summary

Open Cities Health Center is seeking a compassionate and skilled Licensed Behavioral Health Therapist to provide evidence-based mental health services within an integrated primary care setting. The therapist will conduct behavioral health assessments, develop treatment plans, provide individual and group therapy, conduct crisis intervention, and collaborate closely with medical providers to support whole-person care for diverse and underserved populations.

Responsibilities

  • Provide short- and long-term individual, family, couples, and group psychotherapy services using evidence-based, trauma-informed, culturally responsive, and person-centered approaches.
  • Conduct comprehensive psychosocial assessments, behavioral health screenings, diagnostic evaluations, crisis and risk assessments, and DSM-5-TR diagnoses when clinically indicated using validated tools such as PHQ-9, GAD-7, AUDIT, DAST, ACEs, C-SSRS, and other evidence-based instruments.
  • Develop individualized treatment plans, safety plans, measurable goals, and clinical interventions that address mental health, substance use, trauma, crisis needs, social determinants of health, and patient readiness for change.
  • Provide brief intervention services, SBIRT services, crisis intervention, care coordination, and referrals to specialty mental health, substance use treatment, social services, housing supports, and other community resources as clinically appropriate.
  • If licensed as a psychologist, conduct psychological testing and provide clinical supervision to pre-licensed practitioners as needed and within scope of practice.
  • Collaborate with physicians, nurse practitioners, physician assistants, behavioral health colleagues, and interdisciplinary care teams to support integrated whole-person care, treatment planning, consultation, and improved patient outcomes.
  • Participate in weekly group consultation with behavioral health colleagues and attend quarterly all-staff meetings with the entire clinic.
  • Communicate effectively, both orally and in writing, with patients, families, care teams, supervisors, community partners, schools, county agencies, healthcare providers, and other stakeholders as appropriate.
  • Serve individuals from diverse backgrounds and with varied clinical presentations, including patients experiencing trauma, crisis, substance use concerns, co-occurring conditions, and barriers related to social determinants of health.
  • Complete mandated reporting and Duty to Warn responsibilities in accordance with Minnesota law, professional standards, and organizational protocols.
  • Maintain accurate,timely, and legally compliant EMR documentation, including treatment plans, progress notes, assessments, referrals, consultation notes, and other required clinical records.
  • Ensure compliance with HRSA, FQHC, state, federal, payer, HIPAA, and professional requirements whileparticipatingin quality improvement, clinical audits, peer reviews, supervision, trainings, program development, and otherduties asassigned.
  • Knowledge, Skills & Experience
  • Knowledge of evidence-based psychotherapy interventions, behavioral health treatment modalities, integrated behavioral health models, and primary care service delivery.
  • Strong clinical assessment, diagnostic, crisis intervention, suicide prevention, risk assessment, and treatment planning skills.
  • Experience providing trauma-informed, culturally responsive, person-centered care to diverse, underserved, and clinically complex populations.
  • Knowledge of social determinants of health, community mental health resources, referral systems, care coordination, and case management practices that support patient outcomes.
  • Ability to collaborate effectively within multidisciplinary healthcare teams and communicate clearly, both orally and in writing, with patients, families, colleagues, and community partners.
  • Experience using electronic health records and maintaining accurate,timely, legally compliant clinical documentation.
  • Strong organizational, documentation,time-management, care coordination, and follow-through skills.
  • Knowledge of federal, state, and local behavioral healthcare regulations, HIPAA, HRSA, FQHC requirements, payer standards, and professional ethical standards.

Qualifications

  • Master's degree in Social Work, Counseling, Marriage and Family Therapy, or related behavioral health field.
  • Current unrestricted Minnesota license required (LICSW, LPCC, LMFT, or Licensed Psychologist).
  • Minimum of four years of clinical behavioral health experience preferred.
  • Bilingual in Spanish, Somali, Hmong are a plus.
