The Case Manager assists individuals in the Supportive Services program to obtain and maintain permanent, stable housing using housing first principles. Key duties include developing stability plans, managing client data in HMIS, and connecting clients with community resources and public benefits.

Job Summary This position is responsible for assisting individuals enrolled in HOC’s Supportive Services program in obtaining and maintaining permanent, stable housing.
Job Qualifications To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.
to perform this job successfully, an individual must have a Bachelor’s Degree in Social Services or related field and minimum one year of relevant experience in direct client services; OR an AA Degree with 3 years of relevant experience in direct client services; professional experiencing working in low-barrier homeless services or the field of mental health services preferred.

To perform this job successfully, an individual must have a valid Driver’s License and successfully pass background check applicable to position.

Skills: ability to maintain satisfactory relationships with others, excellent customer service skills and a good overall understanding of appropriate human relations. Awareness of and sensitivity to the service population’s culture and socioeconomic characteristics.
Workplace Environmental Conditions While performing the essential duties/responsibilities of this job, the employee will be: Noise Conditions: exposed during a shift to constant or intermittent sounds at moderate to loud levels of sound dependent upon the daily activities.
Candidates must have a Bachelor's degree in Social Services with one year of experience, or an Associate degree with three years of experience. A valid driver's license and the ability to pass a background check are required.
