About the role

Provide support to members to increase employment and independent living skills through partnership with vocational counselors and community providers. Work closely with clinical teams to implement individual service plans and maintain member involvement in social and learning activities.

Join one of our clinical teams as a Rehabilitation Specialist and provide support to our members to help them increase their employment and independent living skills. You will work in partnership with a vocational rehabilitation counselor, community rehabilitation program providers, and other parties to help members increase or maintain involvement in learning, working, and social activities. You will also work closely with the clinical team to ensure all services in members’ individual service plans are implemented.

Requirements

  • Education, Certification, and Experience Requirements
  • Must possess one of the following:
  • Master's or bachelor's degree with a behavioral health or related field
  • Bachelor's degree in a non-related field PLUS one year related experience
  • Associate's degree in any field PLUS one year related experience
  • High school diploma or equivalency certificate PLUS three years' related experience
  • Minimum one year experience working with seriously mentally ill (SMI) adults
  • Experience with vocational, rehabilitation, educational, and social services for SMI adults required
  • Must maintain current CPR, First Aid, and Therapeutic Options certifications (offered through Southwest Network)
  • Must maintain a valid Arizona driver's license and car insurance
  • Must be at least 21 years of age.
