About the role

The specialist assesses and coordinates care for patients referred for mental health services by matching them with appropriate facilities and providers. They are also responsible for maintaining accurate patient records and documentation within the EMR system.

  • They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs.
  • Job Duties/Responsibilities: Review assessment referrals received from various sources in collaboration with the RN and other clinicians, to assess patient needs.
  • Assist in coordinating patient care.
  • Responsible for service coordination by matching patients with appropriate mental health services and providers within UHS, considering factors such as specialty, location, systems complexities and insurance coverage.

Qualifications

  • EDUCATION Masters Degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
  • LICENSURE/CERTIFICATION Preferred Licensed in state, LMSW, LCSW, LPC Current CPR, Vaccines, if required by a customer hospital. (“Customer hospital” means the hospital to which BHC is contracted to provide mobile assessment services.) Must have a valid driver’s license.

Requirements

Candidates must hold a Master's degree in Social Work, Psychology, or a related behavioral health field and possess a valid driver's license. Professional licensure (LMSW, LCSW, LPC) and 1-3 years of related experience are preferred.
