The LPN is responsible for conducting health assessments, monitoring resident behavior, and coordinating the daily administration of medications. They also provide training to support staff and collaborate on treatment plans based on health data and lab results.

Essential Functions and Responsibilities
Complete Health Assessment, Pain Assessment, and Physical for assessing clients
Observes residents to monitor activities and behavior, and documents notable behaviors, medical and medication behaviors, and rules violations. Provide one-to-one supervision for any client at risk.
Coordinates dissemination of medication to clients each day.
Provides support and training for Residential & Milieu Support Staff with regards to medication setup and administration support.
Participate in meetings, track pertinent health data, and collaborate and provide input on treatment plans (i.e., in response to labs).
Conducts regular audits of Medication Administration Records (MARs) to ensure compliance with policies and procedures by non-nursing staff.
During intake assessment, complete the medication review with client and/or families.
