Flexible per diem schedule Choose block scheduling or guaranteed daily hours Competitive pay Mileage reimbursement Independent field-based role with supportive office staff One-on-one client visits Paid documentation time built into your schedule Opportunity to educate and mentor caregivers Family-owned company that values its nurses Help seniors remain safely and independently in their homes Schedule Options Block Schedule Choose the days and hours you're available Paid for your scheduled block of time Our office coordinates client visits, travel, and documentation Guaranteed Hours Guaranteed 5–8-hour workdays available Collaborative scheduling with office staff Typical expectation of approximately four client visits during a five-hour shift, depending on travel distance What You'll Do As a Home Care RN, you will: Complete comprehensive in-home client assessments Develop and update individualized care plans Conduct caregiver orientation and quarterly supervisory visits Educate and support caregivers to ensure quality care Monitor client condition and recommend changes when appropriate Complete documentation within required timeframes Collaborate with office staff to ensure continuity of care Participate in caregiver training and in-service education Perform routine nursing visits as needed, including medication management, blood pressure checks, catheter changes, and other nursing services within your scope of practice Provide hands-on assistance during urgent situations when needed Aging Services Access Point (ASAP) Responsibilities For clients receiving services through Aging Services Access Point (ASAP) programs, you will: Complete initial in-home assessments Develop and update Home Health Aide Plans of Care Match caregiver skills with client needs Conduct caregiver orientation and supervision Complete quarterly supervisory visits and annual reassessments Communicate with ASAP nursing staff regarding client status and care plan updates Report significant changes and advocate for appropriate services
Current Massachusetts Registered Nurse (RN) license Home care, community health, case management, or visiting nurse experience preferred Strong assessment, communication, and documentation skills Ability to work independently Reliable transportation Valid driver's license and automobile insurance Service Area This position serves clients throughout Cape Cod, including: Falmouth Mashpee Sandwich Barnstable/Hyannis Yarmouth Dennis Brewster Harwich Chatham Orleans Eastham Wellfleet Truro Travel is scheduled efficiently whenever possible to minimize drive time.
