Responsibilities

  • Provide direct nursing care to residents in a long-term care setting under supervision and in collaboration with the care team.
  • Monitor vital signs, assess changes in resident status, and report findings promptly to the supervising RN.
  • Administer medications and treatments per physician orders and facility policy; document administration accurately.
  • Assist residents with activities of daily living and mobility; promote safety and fall-prevention practices.
  • Maintain thorough, compliant nursing documentation in the electronic health record; communicate changes to the team.
  • Adhere to infection control, safety, and confidentiality standards; respond to call bells and emergent situations as trained.

Requirements

  • Active New Hampshire LPN license in good standing
  • Graduate of an approved practical nursing program
  • Current CPR/BLS certification
  • Ability to perform standard LTC nursing duties and document care accurately
  • Eligible for travel assignment and able to pass facility orientations and background checks
