We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to join our interdisciplinary team and provide holistic end-of-life care to our patients and their families.

This role involves developing and implementing personalized care plans in collaboration with physicians, social workers, spiritual counselors, and other team members.
This person would be asked to travel to Rockford, McHenry, Hoffman Estates and Huntley areas.
Case management: Coordinate all aspects of patient care, serving as the primary point of contact for patients and families. Comprehensive assessments: Conduct thorough initial and ongoing nursing assessments to evaluate patients' physical, emotional, and psychosocial needs. Care planning: Develop and update individualized patient care plans in partnership with the interdisciplinary group (IDG). Symptom and pain management: Administer medications and treatments to effectively manage symptoms and control pain, ensuring maximum comfort for the patient. Education and support: Educate patients, families, and caregivers on the disease process, symptom management techniques, and end-of-life care. Emotional support: Provide empathetic emotional and spiritual support to patients and their loved ones throughout the end-of-life journey. Oversight and supervision: Supervise Licensed Practical Nurses (LPNs) and certified nursing assistants (CNAs) to ensure quality and compliant care.
Previous experience in hospice care or palliative care. Experience working with individuals who are terminally ill, and their families. Ability to provide empathetic and professional care in end-of-life situations.
Candidates must be a Registered Nurse licensed in the State of Illinois. Previous experience in hospice or palliative care and the ability to provide empathetic end-of-life care are preferred.
