About the role

Coordinates and facilitates patient transfers by collecting clinical information to determine the appropriate level of care. Collaborates with physicians to arrange transfers and maintains updated knowledge of system protocols and governmental data requirements.

Here’s What You Need · Associates Degree in Nursing required · Bachelor’s Degree in Nursing preferred · 4 Years experience as a registered nurse in critical care, emergency services, or case management Req or · 1 year transfer center experience within the last year required · 1 year THR transfer center experience within the last year preferred · RN required upon hire What You Will Do Coordinates and facilitates patient transfers following the Transfer Center's process flow and higher level of care requirements Collects clinical information to determine patient's level of care.

Collaborates with THR on-call physicians and non-THR physicians to arrange transfer.

Attends monthly staff meetings/ be responsible for content discussed during staff meetings.

Other duties as assigned Transfer Center Data Collection.

Collection of specific data points as required by technical software outlined by governmental agencies.

Complete Monthly audits as assigned.

Maintain knowledge base regarding ever evolving changes at each entity.

Follows transfer protocols.

Actively seek out changes and updates regarding the transfer process throughout the system.

Utilizes established protocols and guidelines for ensuring patients are safely and efficiently transferred to the appropriate level of care.

Stay up to date with departmental changes by attending monthly staff meetings.

Complete assigned learning lessons and Touchpoints before their due date Contribute to departmental or organizational projects with approval by Leadership.

Requirements
