JOB DUTIES: Perform physical examinations, obtain medical histories, diagnose illnesses, develop treatment plans, and prescribe medications and therapies in accordance with standardized procedures and applicable scope of practice. Provide preventive and primary care services, including immunizations, health screenings, and wellness counseling. Manage chronic conditions such as diabetes, hypertension, asthma, and depression, with a strong focus on patient education and self-management support. Document all clinical encounters thoroughly and accurately in the Electronic Health Record (EHR) system. Collaborate with an interdisciplinary care team to address medical, behavioral, and social determinants of health. Provide acute care visits and triage support as needed. Support transitions of care, referrals, and follow-up for diagnostic tests and specialty care. Participate in clinical huddles, staff meetings, and quality improvement initiatives. Maintain awareness of and adherence to clinical protocols, policies, and regulatory requirements. Serve as a preceptor for students or trainees when appropriate. Perform other duties as assigned. Document all clinical encounters accurately and thoroughly in the Electronic Health Record (EHR). Participate in clinical huddles, panel management, and quality improvement efforts. Maintain awareness of and adherence to clinical guidelines, protocols, and regulatory standards. Support learners or trainees when applicable. Perform other duties as assigned. Current California Physician Assistant license. Current NCCPA certification. Current furnishing license and DEA certificate (or eligibility to obtain). Current Basic Life Support (BLS) certification. Minimum 1 year of clinical experience preferred (new grads with strong primary care rotations may be considered). Job Preferences: Experience in a Federally Qualified Health Center (FQHC) or community health setting. Proficiency in Epic EHR. Bilingual or multilingual skills.
