LifeLong Medical Care is looking for Family Nurse Practitioners committed to providing high-quality outpatient primary care at our Oakland, CA Health Centers. The ideal candidate must be dedicated to building a collaborative team model of care experience working with the under-served. Case management and behavioral health services are fully integrated into our care to address various medical and psychosocial factors impacting the patient's overall health. The Health Center utilizes a multidisciplinary team approach with primary care providers, RNs, Social Workers, Psychiatry, Podiatry, etc. As a NP/PA at our Health Center, you'll have the opportunity to work alongside a multidisciplinary team of healthcare professionals to provide comprehensive care to our patients. Our Health Center utilizes a collaborative team model of care, integrating case management and behavioral health services to address the various medical and psychosocial factors impacting our patients' overall health. JOB DUTIES: Perform physical examinations, obtain medical histories, diagnose illnesses, develop treatment plans, and prescribe medications and therapies in accordance with standardized procedures and applicable scope of practice. Provide preventive and primary care services, including immunizations, health screenings, and wellness counseling. Manage chronic conditions such as diabetes, hypertension, asthma, and depression, with a strong focus on patient education and self-management support. Document all clinical encounters thoroughly and accurately in the Electronic Health Record (EHR) system. Collaborate with an interdisciplinary care team to address medical, behavioral, and social determinants of health. Provide acute care visits and triage support as needed. Support transitions of care, referrals, and follow-up for diagnostic tests and specialty care. Participate in clinical huddles, staff meetings, and quality improvement initiatives. Maintain awareness of and adherence to clinical protocols, policies, and regulatory requirements. Serve as a preceptor for students or trainees when appropriate. Perform other duties as assigned. JOB DUTIES: Obtain complete medical histories and perform physical exams. Diagnose illnesses and injuries and develop appropriate treatment plans. Prescribe medications and therapies within scope of license and in compliance with LifeLong protocols and state regulations. Provide preventive and chronic care services including immunizations, screenings, and patient education. Order, interpret, and follow up on diagnostic tests and referrals to specialty care. Collaborate with behavioral health, nursing, case management, and support staff to deliver coordinated, team-based care. Document all clinical encounters accurately and thoroughly in the Electronic Health Record (EHR). Participate in clinical huddles, panel management, and quality improvement efforts. Maintain awareness of and adherence to clinical guidelines, protocols, and regulatory standards. Support learners or trainees when applicable. Perform other duties as assigned. QualificationsJOB QUALIFICATIONS: Strong clinical skills with a demonstrated commitment to providing care in underserved communities. Ability to work independently and collaboratively within a care team. Culturally sensitive, with excellent communication and patient engagement skills. Organized, reliable, and adaptable in a fast-paced, mission-driven environment. Job Requirements: Graduate of an accredited Physician Assistant program. Current California Physician Assistant license. Current NCCPA certification. Current furnishing license and DEA certificate (or eligibility to obtain). Current Basic Life Support (BLS) certification. Minimum 1 year of clinical experience preferred (new grads with strong primary care rotations may be considered). Job Preferences: Experience in a Federally Qualified Health Center (FQHC) or community health setting. Proficiency in Epic EHR. Bilingual or multilingual skills.

Requirements

Candidates must be graduates of an accredited Physician Assistant program with a current California license, NCCPA certification, and BLS certification. At least one year of clinical experience is preferred, though new graduates with strong primary care rotations may be considered.
