We are looking for a proactive employee to provide person centered services and support to assist individuals in achieving independence while maintaining their health and safety. Responsibilities include: Supporting participants with activities of daily living (ADL) Implementing participants behavior guidelines and goals as written in Staff Action Plan Monitoring health and safety, building advocacy, decision making, and community connections Teaching participants skills to promote independence Education/Experience: High school diploma/GED and experience working with people with disabilities preferred. Basic word processing and e-mail skills are required.
