What You'll Do ✅ Diagnose and treat acute and chronic conditions ✅ Provide care in homes, Assisted Living Facilities (ALF), and Skilled Nursing Facilities (SNF) ✅ Conduct patient assessments, develop treatment plans, and prescribe medications ✅ Utilize telehealth and in-person visits ✅ Collaborate with patients, families, and healthcare partners ✅ Maintain accurate and timely clinical documentation
