About the role

The Hospice RN Case Manager is responsible for performing patient assessments, developing care plans, and managing symptoms for patients in King County. They provide emotional support for end-of-life care and coordinate with interdisciplinary teams and outside services.

Due to the growth and opening of new territories we are hiring a Hospice RN! Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN) in King County, Washington (Federal Way area)!

Requirements

Candidates must be a licensed Registered Nurse with current BLS certification and a valid driver's license. Prior experience working in a hospice agency is strongly preferred.
