Delivers patient-centered clinical pharmaceutical care and medication therapy management for chronic inflammatory diseases, HIV, and oncology. Coordinates with healthcare providers to implement care plans while ensuring compliance with safety and privacy regulations.
This position requires flexibility to orient and work at provider clinics and specialty pharmacies.
Clinical knowledge and therapy management for Chronic Inflammatory disease states/HIV/Oncology.
Maintain excellent written and verbal communication skills and strong critical thinking skills.
Excellent organizational skills, and ability to prioritize/follow-up.
Flexible; able to cope with rapid change; comfortable with an ambiguous environment.
Ability to troubleshoot problems efficiently and effectively.
Ability to provide comprehensive medication management including initial assessments, toxicity evaluations/symptom management, and reassessments.
Capability to set priorities amongst a series of competing clinical and operational priorities and to effectively coordinate and direct the work of others to meet specified deadlines.
Proficient computer experience and accurate data entry skills in MS Word and Excel.
Experience as a specialty pharmacist in a hospital setting.
Board Certification (ex: BCACP/BCPS/BCOP/CSP)
EPIC and Beacon experience.
ASHP-Post Graduate Year 1 ASHP General Pharmacy Residency.
EDUCATION PHARMD-Graduate of an accredited pharmacy school CERTIFICATION & LICENSURE PHARMR-Current registration or Registered Pharmacist within 120 days
