Responsibilities

Job Summary Duties to include but not limited to: the assessment/evaluation of patient withdrawal followed by the execution of appropriate medication detox protocols; administration of prescribed medications and patient education.

Provide patients with a safe medically managed detox by assessing signs and symptoms of withdrawal. Monitors the effects of medications administered and documents accurately and appropriately. Reports and documents medication errors, pharmacy errors, packaging errors, medication count concerns, etc. Ensures that ongoing assessments are conducted as determined by the patient’s condition using COWS & CIWAS and re-prioritizes care accordingly. Maintains neat and accurate narcotic control inventories and all documentation related to the administration of medications. Promotes patient's independence by establishing patient care goals, teaching patient to understand condition, medications, and self-care requirements, answers patient questions. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations. Resolves patient problems and needs by utilizing multidisciplinary team strategies, including de-escalation techniques. Documents patient care services by charting in EMR. Maintains continuity among teams by documenting and communicating actions, irregularities, and continuing needs. Contributes to team effort by accomplishing related results as needed. Participates in the onboarding of new nurses for medication administration. At end of shift communicate to incoming LPN pertinent data regarding patient activity and conducts medication count. Treats patients, and colleagues with dignity and respect; honors each person’s right to privacy/maintains very strict patient and colleague confidentiality; respects all individual differences; supports cultural diversity. Knows role and responds appropriately to all codes related to emergency situations. Consistently arrives to work on time and adheres to attendance policies. Other duties, as needed.

Qualifications

Education/Experience: Current LPN License in State of operations. LPN Diploma & State License Required Knowledge of drug and alcohol treatment preferred. Actual compensation within this range will depend on the candidate’s skills, experience, education, and relevant qualifications. Generally, placement within the upper portion of the range is reserved for candidates with several years of directly related experience or exceptional expertise.
