About the Job: We’re looking for a Family Nurse Practitioner to join our collaborative team.
You’ll focus on early intervention, chronic disease management, and long-term wellness in a setting that values quality over volume.
What You’ll Do (Key Responsibilities in Patient Care): Comprehensive Care: Provide both in-person and virtual visits to increase access and continuity of care.
Diagnosis & Planning: Prescribe medications and develop personalized health plans with your patients.
Wellness Education: Educate patients on wellness, prevention, and lifestyle management.
Collaborate closely with physicians, medical assistants, and other clinicians.

Requires an active and unrestricted FNP license, board certification, and DEA registration. Preference is given to candidates with 2+ years of primary care experience treating patients aged 2 and older.
