About the role

The Case Manager is responsible for managing foster care placements, ensuring regulatory compliance, and conducting regular file audits. They coordinate child/family team meetings and ensure youth receive necessary medical and mental health services.

Responsibilities

  • Case Manager - Foster Care Program The ideal candidate must reside in the Northwest/Middle TN area of the state: Montgomery, Cheatham, Stewart, Robertson, Sumner, Davidson, and Dickson counties.
  • Universal Health Services, Inc. company, in partnership with Greystone Foster Care, has a long history in providing comprehensive behavioral health care services to at-risk youth struggling with extensive mental health issues.
  • Greystone Foster Care recruits foster parents and offers the training to be equipped to provide a safe, stable and therapeutic environment for children and youth that have been removed from their homes.
  • Our training prepares staff and foster parents to support children and youth as they begin the healing process.
  • Our therapeutic foster parents link children and youth to hope, healing and trust.
  • Greystone Foster Care is seeking a dedicated, compassionate Case Manager to add to our growing team.
  • This is a hybrid job blends remote work and in-office position with occasional requirements to report to the facility.
  • This role requires full-time dedication and must be flexible to include weekends and evening work.
  • Job Duties/Responsibilities: Complies with state regulatory requirements and Greystone Foster Care plan Ensures that all placement documentation is completed according to established timelines Completes regular file audits for each child and participates in regular peer file audits to ensure audit preparedness at all times Assists with completing foster parent reassessments when necessary Schedules child/family team meetings at least quarterly and corresponding documentation is completed/submitted Visit each assigned child per policy or as needed Ensures that annual EPSDT and biannual dental appointments are scheduled and information provided to DCS Ensures that outpatient mental health assessments are scheduled for all Level 2 youth and participation in recommended services occurs Obtains required consents and releases from appointed guardians for medical appointments and medications Benefits Highlights Challenging and rewarding work environment Competitive Compensation & Generous Paid Time Off Mileage Reimbursement Stipend cell phone funds Excellent Medical, Dental, Vision and Prescription Drug Plans 401 (k) with company match and discounted stock plan SoFi Student Loan Refinancing Program Career development opportunities within UHS and its 300+ Subsidiaries!

Requirements

  • Bachelor's degree in Social Work or related field; Master's degree preferred One to three years' related experience and/or training, or equivalent combination of education and experience Valid state Driver's License Automobile insurance with verification of medical liability. ***This role requires a successful background check, fingerprinting and employment verification**
