Active CNA license Minimum 3 years of professional experience in dementia or memory care Ability to lift at least 35 lbs. Companionship & Engagement: Foster connection through conversation, games, music, and mentally stimulating activities tailored to the individual. Daily Living Assistance: Support clients with personal care needs such as bathing, dressing, toileting, mobility, grooming, and feeding. Light Housekeeping & Errands: Assist with meal prep, laundry, cleaning, and transportation as outlined in the care plan. Monitoring & Communication: Observe and report changes in client condition, behavior, or cognition to the Care Coordination Team. Complete documentation after each shift. Make a Difference with SAGE!
