Provide direct, person-centered care and support to individuals with disabilities or special needs in their home environment Assist clients with activities of daily living (ADLs) including bathing, grooming, dressing, and meal preparation Support clients in community integration, social activities, and skill-building exercises Implement individualized support plans and document progress accurately Administer medications and monitor health conditions as directed Communicate effectively with supervisors, family members, and care team members regarding client needs and progress Maintain a safe, clean, and supportive home environment for clients
