  • Type: Full TimePOSITION SUMMARY Position will provide a wide range of case management services for people infected with HIV. Serves as an advocate for the client and their family. Will promote available services for the client within the agency and will refer the client for services outside the agency. Coordinates intakes, establishes eligibility and distributes housing funds for HOPWA. Will maintain a flexible schedule in order to facilitate client care, will do referrals to housing and outreach for other services.

Does initial intake and yearly update paperwork for all clients who are eligible for housing subsidies Inspects and updates client charts as needed for all housing programs Inspects apartments to assure that funding source standards are met Submits monthly and quarterly HOPWA lists to the business office for rent payment Files annual and bi annual HOPWA. Coordinates emergency HOPWA and Emergency Shelter payments Participates in monthly Homeless consortium Monitors monthly housing allowance to stay within budget Inspects housing documentation completed by other case managers for accuracy Works with the Longview City Housing Office to keep housing slots full at all times Accompanies clients to the City Housing office, serving as an advocate Completes required matching funds reports *Provides a range of client-centered services that link client with health care, psychosocial and other services *Case Management provides initial comprehensive assessment of the clients needs and personal support systems Development of a comprehensive, individualized service plan Coordination of the services required to implement the service plan Periodic evaluation and revision of the service plan as necessary over the life of the client. Client specific advocacy and/or review of utilization of services. Prepares confidential and non-confidential correspondence in a professional style and format. Inputs data into databases correctly and within designated time frame. Makes appointments and referrals for clients as needed. Maintains effective filing system and files necessary information in a timely manner. Complies with DSHS and other funding source’s standards for charting and case management Records client sensitive information using approved methods. Assists supervisor in preparing programmatic reports in an accurate and timely manner. Communicates readily with Supervisor on programmatic, agency interpersonal, and significant client information. *Communicates with all community members: such as clients, general public, and other services providers in a friendly, cooperative and professional manner to ensure non-duplication of services. Prepares monthly reports in a timely manner as requested by supervisor. Prepares time reports and mileage accurately and on time. Serves as an advocate for the clients at all times, avoids derogatory and/or inappropriate remarks regarding clients. Ensures that all services given to clients are based on client eligibility guidelines. Must be familiar with all currently approved service caps. Makes referrals to social service agencies that provide services not obtainable from Special Health Resources. Ensures that all clients have accurate and up to date consents and forms in charts. Must be familiar with all complaint procedures. Must be familiar with ALL procedures and forms in the HOPWA manual and have all forms accurately filled out. Conducts self in a manner consistent with professional standards. Adheres to all Agency Personnel and Program policies.

  • QualificationsQUALIFICATIONS: Minimum B.A. in a human service field or 2 years’ experience in social services or equivalent KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED Good multi-tasking ability. Good written and oral communication skills Computer literacy Able to schedule work activities according to priority. (Strategic planner) Ability to complete documentation in legible and professional manner (All interaction with clients) Needs to be familiar with community resources and be able to direct persons to the appropriate services. (Networking skills) Must be able to make independent day-to-day decisions. Needs to be solution-based person with strong problem-solving ability. Must work as a team player with other staff. Must take direction in a professional manner Must have the ability to maintain the maximum level of confidentiality. Ability to make sound judgment calls in crisis situations. Requirements

Requires a minimum of a Bachelor's degree in a human service field or two years of experience in social services. Candidates must possess strong communication skills, the ability to maintain confidentiality, and familiarity with community resources.
