Public Health Nurse I or II - Maternal and Child Health (Beautiful Beginnings Program) (Grant-funded position)

The Public Health Nurse I or II works directly with historically marginalized individuals, carries out health promotion programs at all levels of prevention, conducts basic data collection and analysis, and participates in field work including home visits and other outreach activities. Specifically, this position provides personalized case management to individuals and their infant(s) during pregnancy, the postpartum period, and/or up to the child’s third birthday. This position is grant funded through a Community Partnership Grant.

Duties

Include:

Provides comprehensive health care coordination to pregnant and postpartum individuals along with their families.

Conducts home and/or community visits with families and children from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds.

Evaluates and performs physical nursing assessments with individuals and their infants.

Creates a care plan for the client and their infant(s) based upon any stated and observed needs.

Uses data related to the social determinants of health to provide and coordinate care for all clients.

Provides education and appropriate resources to clients, families, and community members pertaining to health conditions and other relevant public health topics.

Accurately documents all case management and client contact in an Electronic Health Record System including using mobile technology for partial remote work and on home visits.

Participates in other related duties as assigned.

Requirements

  • Public Health Nurse I
  • Associate degree in Nursing from an accredited College of Nursing
  • Knowledge and proficiency using an Electronic Health Record system.
  • Current and unrestricted Ohio Registered Nurse license; Ohio Driver's License; BLS CPR Certification
  • Bachelor's Degree in Nursing Preferred.
  • OBGYN, postpartum, or related experience preferred.
  • Lactation counselor certification preferred.
  • Public health or related nursing experience preferred.
  • Case management and home visiting experience preferred.
  • Public Health Nurse II
  • Bachelor's degree in Nursing from an accredited College of Nursing
  • 2 years’ experience in nursing including OBGYN, postpartum, public health, case management, home visiting, or related.

  • Current and unrestricted Ohio Registered Nurse license; Ohio Driver's License; BLS CPR Certification

Hiring Salary

  • $28.38/hour - $33.35/hour for Public Health Nurse I
  • $30.24/hour- $35.53 for Public Health Nurse II. This is a non-exempt position.
