As the Family Support Service Group Facilitator, you will play a pivotal role in individuals' lives, making a significant impact through:
Plan and facilitate monthly family/peer support groups including a meal or snack and an educational component
Accept and track referrals for the support groups from other human service providers, SPOA, FOCUS, county agencies, including the Office of Community Services, and individuals and/or families who self-refer, ensuring families and children have the opportunity to participate in support groups
Maintain up-to-date documentation for all support groups and outreach, including who attended, what topics were discussed, what activities were done, if there were any guest speakers, date, time, and location of the event
Provide outreach regarding the Family Support Services support groups as approved by the Program Director
Submit monthly documentation of services to the Family Peer Advocate
