Harbor is seeking a Psychiatric Nurse with an LPN license to join the Toledo team! The LPN is responsible for providing nursing care to clients in outpatient mental health and substance use treatment settings while utilizing the nursing process of assessment, planning, intervention, and implementation.
Position is full-time and 1st shift.
Education/Experience/Other Requirements
- Current unrestricted LPN license from the state of Ohio.
- CPR/First Aid and NCI/CPI certifications required within 90 days of employment.
- Must maintain active licensure in good standing.
- Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier
Essential job Competencies/Primary Duties
- 1.
- Provide direct nursing care to patients with mental health and substance use disorders in an outpatient setting.
- 2.
- Conduct patient intake, including vital signs, medical/psychiatric history updates, and screening assessments.
- 3.
- Assist providers with psychiatric evaluations, procedures, and medication management visits.
- 4.
- Administer medications as ordered, including oral and injectable treatments, and monitor for side effects or adverse reactions.
- 5.
- Educate patients on prescribed medications, treatment plans, and symptom management strategies.
- 6.
- Perform patient triage, including assessing urgency of needs, responding to patient concerns, and coordinating timely care.
- 7.
- Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).
- 8.
- Collaborate with providers, supervisor, therapists, case managers, and medical assistants to ensure coordinated, patient-centered care.
- 9.
- Support crisis intervention efforts using appropriate de-escalation techniques and seek additional clinical support as needed.
- 10.
- Coordinate care activities such as scheduling, referrals, prior authorizations, and follow-up communication.
- 11.
- Ensure adherence to all regulatory, safety, and infection control standards, including patient confidentiality requirements.
- 12.
- Contribute to efficient clinic operations by managing workflow, supporting patient flow, and completing administrative tasks as assigned.
- 13.
- Utilizes information technology systems and software applications effectively and in accordance with organizational standards and security protocols.
- 14.
- Demonstrates sound judgment by analyzing information thoroughly, reaching timely conclusions, and taking appropriate, decisive action.
- 15.
- Plans, prioritizes, and organizes work to ensure maximum efficiency, productivity, and quality outcomes.
- 16.
- Actively seeks and utilizes supervision and feedback appropriately to support professional growth and effective job performance.

- Manages time and utilizes leave in accordance with organizational policies and procedures.

- Performs additional duties and responsibilities as assigned to support departmental and organizational needs.

- May provide billable services and may be required to meet agency productivity expectations.

- Travel to other locations may be required to meet the operational needs of Harbor.

- Must maintain strict adherence to HIPAA standards at all times to ensure patient confidentiality and privacy 22.
