The Licensed Practical Nurse (LPN) Care Coordinator provides comprehensive care coordination services to FellowshipLIFE members and clients across home and community-based settings. and serves as the primary point of contact for members and families, ensuring seamless coordination of healthcare services, community resources, and interdisciplinary communication to promote safe, high-quality, person-centered care.
Coordinate services across the healthcare continuum to ensure timely, efficient, and member-centered care.
Serve as the primary contact for assigned members and caregivers.
Assist the RN Care Manager with implementation and monitoring of individualized care plans.
Perform ongoing telephonic and in-person follow-up with members to monitor changes in condition and service needs.
Coordinate physician appointments, specialist referrals, diagnostic testing, transportation, errands, and community resources.
Facilitate smooth transitions following hospitalization, emergency department visits, skilled nursing stays, rehabilitation, or hospice services.
Assist members and families in navigating healthcare services and available FellowshipLIFE programs.
Identify barriers to care and communicate concerns promptly to the RN Care Manager.
