Position Summary

The Homeless Outreach Clinical Case Manager is responsible for engaging individuals experiencing homelessness through street outreach, shelters, encampments, and community locations. This position provides comprehensive case management and clinical services that promote housing stability, self-sufficiency, and improved quality of life. The Clinical Case Manager works collaboratively with community partners to connect clients to housing opportunities, healthcare, behavioral health services, and other essential resources.

Duties and Responsibilities

Conduct outreach and clinical assessments to individuals experiencing homelessness in shelters, encampments, and community settings. Build trusting relationships through trauma-informed, client-centered engagement. Complete comprehensive assessments to identify client needs, strengths, and barriers. Develop individualized service plans with measurable goals. Provide ongoing case management clinical services and monitor client progress toward established goals. Connect clients with medical care, mental health services, substance use treatment, and public benefits. Assist clients in obtaining identification documents, Social Security benefits, Medicaid, SNAP, and other assistance programs. Advocate for clients with landlords, housing providers, healthcare agencies, and community organizations. Maintain accurate, timely, and confidential client records in accordance with agency policies and HMIS documentation standards. Participate in case conferences, multidisciplinary team meetings, and community outreach events. Maintain knowledge of community resources and referral networks. Respond appropriately to crisis situations while ensuring client and staff safety. Perform other duties as assigned.

Qualifications

Relevant experience in homelessness services, case management, behavioral health crisis assessments, comprehensive psychiatric evaluations Experience working with individuals experiencing homelessness, mental illness, substance use disorders, or trauma preferred. Strong crisis intervention and de-escalation skills. Excellent communication, organizational, and interpersonal skills. Ability to work independently and collaboratively with multidisciplinary teams. Proficiency with Microsoft Office and electronic case management systems (HMIS preferred). Valid driver's license and reliable transportation is required.

Education

Master’s degree in social work, Human Services, Psychology, Sociology, or a related field preferred.

Knowledge, Skills, and Abilities Understanding of homelessness, poverty, and systemic barriers affecting vulnerable populations.

Knowledge of local community resources and social service systems.

Ability to establish rapport with diverse populations.

Strong problem-solving and advocacy skills.

Ability to maintain professional boundaries and confidentiality.

Cultural competence and sensitivity when working with diverse communities.

Excellent documentation and time management skills.

Working Conditions Frequent travel throughout the community, including shelters and encampments.

Regular walking and standing, Evening or weekend hours may be required.

Exposure to various weather conditions and community environments during outreach activities.
